My Story
The service unit, subsidised by the Hospital Authority, was established in 2011. It operates in Wan Chai and the Eastern District, serving the Ruttonjee Hospital and the Eastern Hospital. Through a multidisciplinary team (comprising social workers, nurses, physiotherapists, occupational therapists, speech therapists and care workers), the unit provides appropriate, caring and professional home-based, rehabilitation and community support services for discharged elderly persons. This assistance aims to facilitate post-discharge emergency support, adaptation, rehabilitation and caregiver support to reduce the risk of readmission.
